Medical Homebound Request Process


1. Complete the Parent Expectations for Homebound Services form and bring the completed form to the Spartanburg School District 2 Administrative Office (3231 Old Furnace Road, Chesnee, SC 29323). This form is available on the D2 website, your student’s school, or the District Office. This form is required and must be completed in full and signed by the student’s parent/guardian in order to proceed with the Medical Homebound Request process. It is also advised that you read the Homebound Instruction Guide before proceeding with the request. 

2. Once the Parent Expectations for Homebound Services form has been completed, signed and brought to the D2 District Office, parents/guardians will be given the Medical Homebound Instruction Form to be completed by the student’s physician. The Medical Homebound Instruction Form can only be picked up, in person, at the district office and will only be given to parents/guardians that have completed the Parent Expectations for Homebound Services.

3. Medical Homebound Instruction Form Section III – REQUIRED RELEASE is required to be signed and dated by the parent. If this portion of the form is not completed, the Medical Homebound Request will NOT be considered.

4. Take the Medical Homebound Instruction Form to the student’s physician or mental health provider and ask them to complete SECTION II – MEDICAL INFORMATION.

5. The completed Medical Homebound Instruction Form must be faxed by the student’s physician to 864-515-5198 or 864-578-8924. Once the Medical Homebound Instruction Form is faxed and received, the Director of Special Services will review the Medical Homebound Request.

6. The Homebound Coordinator at your student’s school will contact you if your student is approved for Medical Homebound Instruction. If the Medical Homebound Request is denied, you will be notified via phone by someone at the District Office.

Translations

Information displayed directly on our websites can be translated to a number of languages by clicking on the "language" drop down menu in the top left corner of any page. 

PDFs or other documents on the website can easily be translated by following these directions. 

  1. Download the document you want to translate to your computer.
  2. Access the Translate Document tool via Google Translate.
  3. Choose the language you would like to translate from and to. If you’re not sure what language your PDF file is in, you can set the input language to “Detect language.”
  4. Upload your file to Google Translate by dragging/dropping or browsing your files. Then click "translate".
  5. Google will then translate the text.
  6. You will then get a pop-up with the PDF file translated.
